This is a reminder that Vulnerable Sector Checks (Police Checks) need to be completed every 5 years by all adults (18 and over) who will be working with children involved in Wingham Minor Soccer.  Chances are, if you don't know what we mean, or you can't remember if you have done one recently, you should get one done.

Already have one completed?  Skip to the bottom on how you can get it to us.  Otherwise, follow the instructions below.

STEP 1 - GATHER ALL REQUIRED FORMS AND ID

1. Wingham Minor Soccer Volunteer Form - Click here
This will ensure you get your police check done for free - instead of the $20.00 fee

4. Two pieces of valid ID
Please read the following regarding ID needed for Vulnerable Sector Check

Accepted Photo Identification (1 of your 2 pieces of identification must be from this list):

  • Driver's Licence
  • Passport 
  • Ontario Photo Card
  • Government Employment Card (that contains both your name and date of birth)
  • Military Employment Card (that contains both your name and date of birth)
  • Age of Majority Card
  • Canadian Citizenship Card (up-to-date)
  • Indian Status Card
  • International Student Identification Card (ISIC)
  • Permanent Resident Card
  • Possession and Acquisition License (PAL)

Accepted Non-Photo Identification (must contain both name and date of birth):

  • Birth Certificate
  • Baptismal Certificate
  • Hunting License
  • Fishing License
  • Outdoors Card
  • Hospital Card
  • Immigration Papers (Work permit, Study Permit, Refugee Claimant, etc)

Important Notice: Ontario Health Cards cannot be accepted for identification purposes (Personal Health Information Protection Act, 2004 section 34(4)).


STEP 2 - SCAN AND SEND INFORMATION

STEP 3 - WHEN NOTIFIED, PLEASE PICK UP YOUR PRINTED VULNERABLE SECTOR CHECK AT THE CLINTON OPP STATION at 325 Albert St, Clinton, ON N0M 1L0


STEP 4 - DELIVER TO THE WMS EXECUTIVE TWO WAYS:
You can get it to us two ways:
1. Scan and send via email to [email protected]
2. Make a paper copy and send it to Marc Westra: POBox 341 Wingham, ON N0G2W0
